The Diabetes Care Program is a caring partnership that promotes the prevention and treatment of diabetes. Recognized for excellence by the Indian Health Service, the program emphasizes partnership, education, and healthy living to help manage diabetes and improve overall wellbeing.

Who Can Use It
Tulalip tribal members or members of any federally recognized American Indian/Alaska Native tribe residing in Snohomish County.

How to Join

Costs
No cost to eligible members.

What to Expect
You’ll work with a team that offers personalized education on managing diabetes, including one-on-one support, diabetes classes and holistic healing. Continuous glucose monitoring (CGM) and nutrition services are also available. Wisdom Warrior classes offer help for chronic disease management, and there are community activities like gardening, cooking, and wellness walks on local trails—all designed to help you create a healthier lifestyle in a supportive environment.
