What to Expect
The Community Health Department offers a range of programs that support every stage of life. A Resource Specialist will guide you through the health system, help with medical needs, and provide self-care tips. If you’re in the hospital, the Hospital Liaison helps with your discharge plan and connects you to local support.
For general health, the Community Health Nurse Program provides education, vaccination clinics, and follow-up care. The Prevention Education Program offers resources like a Nicotine Replacement Therapy program. Families can also use WIC for nutrition, breastfeeding support, and referrals to other resources.
