Description
Tulalip Community Health Nurses educate and care for individuals and families to enhance the spiritual, emotional, physical, and environmental health of Tribal members and their families. Nurses are partners in creating the highest levels of well-being.
Some services include training in critical first aid; care coordination to help people navigate the health system and advocate for you during hospital stays and for care such as dialysis. Nurses can also come to the home to provide care and make sure the environment is safe. Elder Wellness Services provides elders with health monitoring and engagement with visits, check-ins and care coordination. Community Health Nurses also run the Container for Life program, which helps emergency services personnel quickly assess and respond in emergency situations.
Who Can Use It
Tulalip tribal members and their families. Care coordination is available to all Tribal members in Snohomish County. Elder services are available to all Tribal members aged 57 and older.
